Registration Mod possibility?

I've made several failed attemps to reproduce this registration form.

Registration mod

The Admin of this site was none to friendly or helpful, SMF seems un-intrested as well,
so I look to my new friends here at TP.
Any ideas?

I have registered on that forum and waiting for a reply to MOD request.

The Custom Profile Field mod is not really related to what you are looking for.  It allows you to add custom fields in every user's profile that can be required, or not (as you configure) to be filled in during registration.  But you are limited in the extra information that you would want to display.  If you have a limited amount of questions, and description, it might suffice but looking at the example you linked, it seems you want a bit more elaborate type of setup.

The Custom Profile Field mod does not have radio button controls but you could use the selection box in its place.  However, what is it you wish to be displayed with this info?  If you want a long description area or areas and customized, a registration mod would probably be better.

How do I get this javascript to run on the Registering
part of my site? I edited  Registration Agreement in the admin panel, cut and pasting the code as is.

Would any one like to help me get this javascript working in my Registration Agreement?
