Blank page on poll vote

I was using the recent polls from forum block (SSI tp module) and for some reason when anyone trys to vote it looks like they get an SSI error and the page "....SSI.php?ssi_function=pollVote" just appears blank.  Anyone ever have this problem?  I checked error log but don't see anything related to poll vote.

Does anyone know which file the Poll TP module is in?  I want to try overwriting it or at least taking the code from it.
