You know, all along I could have just gave you a copy of my default theme file.  but, that would be too easy. Here you go, this should work. You were missing this part.

Everything is CHMODed to 777...

This is the default theme right?
Only thing that I can think of that would make it a blank page is permissions or corrupted files. You check all the files for SMF/TP to make sure they weren't 0 in size and stuff?
