submit form to db

Hi guys, im no php programmer at all but I can stumble along through it ok most times.

I have what's hopefully a very simple noob question.

I basically what to put a form with a text box and a couple of option boxes in a php block and submit it to a table in my main smf database. I can read and write from the database ok without problems i am just not sure about dealing with the form in a php block? I was looking at the generic application block code but its so loaded with options that its been hard for me to digest the basic simple elements of the form submission.  I am using SMF 2 and TP 1 for what its worth.
