Edit Registration Page

I would like to edit the registration page of my forum to include more information.  I have no programing skills and am shooting completely in the dark.  Currently the page asks for username, email, and password.  I would like to add Name, Phone number, address and zip code. 

Any help with this will be greatly appreciated.  I thought by adding a portal I would have greater functionality with the registration page but I cannot see how to do it.

Your question is actually an SMF question because the registration page is an SMF file so you should ask your question there.
If you search you should find that it's already been answered there.
