There would be no distortion if css background property is used for them - but there will be cropping, which might actually not look so bad.Extracting the first [img] tag is trickier and will slow down the script, likewise adding custom option for every topic - that rather belongs in a better frontpage manager/module for TP where for example caching would be done to minimise this. In addition also mixing in RSS feeds to supply the text/images. As stated elsewhere I have plans for such a module.

I will investigate how much is involved in adding this option for "forum only" for frontpage though, as its already fetching the authors avatar for each topic, adding the first attachment will not mean adding too much overhead I suspect.
