Upgraded from 1.0.3 to 1.0.4.  This upgrade did not affect the Simplicity index.template.php, which stayed at 1.0.  I guess this is okay?

hmm what you could do is make a back up of that file, uznip the update, and then over write that file to get it to the right version. If this messes anything else, just use that back up of the file :).

The version number doesn't matter that much. its more a reference than a actual indication. The main thing is if the theme works as before and do not produce new errors.

Even the default theme have different versions numbers in some template files. Those are actually more important - but only in the sense that you can see which files have changed much(newer numbers) and which have stayed untouched(probably lower versions numbers)
