could be another mod has altered the code in your index.template, do you have any other mods installed? and are you using the default theme?
Sometimes you have to install some mods in the correct order, and other times it's a permissions thing. You should create a new topic too.
It helps keep things orginized. ;D
Check and make sure your folders and files are at least 644.
If that doesn't work, CHMOD to 777.
EDIT: That sounds bad. Make sure your MOD folders are at least 644. Not all of them. You should NEVER CHMOD all files and folders to 777. Bad thing to do.
Yes, it's repairable.
SMF has a secret hidden up it's sleeve if you know where to look, and as one SMF support member told me.... SMF is smart! I have to agree.

you will see backups. Choose the latest one which should have a name that sounds kind of like what was happening before the mod like Before_TinyPortal or whatever the file you have is called.
Download that and decompress it. Then upload all those files to your site. They are all the SMF files before the installation of the mod.
Just passing on some knowledge, hope this helps you get back to a point where you can start again.
You have to unstall the mod the sameway you install them on your forum.
This happen to me before. And I unstall my mods the same way I install them. After I done that I install TP and all the tested pass. After that I install all my mods back on my forum.
You should try to unstall the mods in the same way you install them on the site. To see this will sovled your error tp.
Edited --
In TP have a Shoutbox so you dont needed that mod for the SMF Shoutbox after you install TP....Just letting you know.
You dont have to uninstall anything. There are more then one mod of yours that change the index.template.php in the same places as TP.
Download the manual install package and upload all the files manually, and follow the mod instructions here

can i install the mod using package manager regardless of the 'index.template.php' error and make the necessary changes to this file manually?
You can.I installed it with that error and so far so good.I haven't had any issues yet.
But you should do what Shadow Queen has done,so you can avoid the error all together
If that wont work try to do a manual installing of TP ok. But try that first what I have done ok
If you uninstall the mods all of the info stays in the database. When you reinstall all of the info should still be there. And as far as the order I install them I usually go by the order of importance Meaning I install the one that are most important to me first.
YOu dont lost no data at all. All the mods data and more will be in the database ok.
@ Shadow Queen
It's really great you want to help us with support here, but you really dont have to echo an answer already given by the poster before you.
I have seen this in several places now, and you really dont have to give the same answer again. ;)
But nobody says it quite like shadowQ ;)
@G6 -- I will try my best to stop that. Sometimes I will do that for no reason at all. Im sorry, Its hard for me to stop doing dump things like that. :-\ I hope you do understand G6 :) And thank you for that nice comment you made :)
