First off. NEVER install ANYTHING to your forum without using the DEFAULT SMF CORE theme.
You seem to NOT ising that thrme as default during the install from what the screenshot is showing.

Often the index.template.php gives you errors when you have other modifications installed and who also uses and  have changed the lines the installed is searching for, if the lines dont match cause the other mod have changed them, you get a test failed.
So if you have other mods installed, uninstall them and try the packagemanager again for TP.

If you still have errors, download the premade zip file from this board sticky post, and tale out the index.template.php file from it, and go ahead with the insaller even if you get an error, then replace the old index.template.php with the one from the zipfile.

Doesent matter, it's just the theme it copies over, not the entire SMF setup.
Thats why it is so important to actually follow the guidelines the developers say, they shoud know best wouldent they  ;)
