It's likely some conflict with another modification you have installed.
I would go ahead and install it. Then download the "files_only" package and look for the Mod_instructions_SMF11rc.html file. Download your index.template.php file from your server. Make a backup of it in a safe place. Apply the mod instructions and upload the file into your Themes/default directory. You should be good to go.
