Article is truncating and I can't fix it!

I created a form that works fine.  I loaded it into an article and the editor keeps truncating the code and has an overflow problem just like http://www.tinyportal.net/index.php?topic=12577.0.

I'm a newb, so I didn't recognize the $t_out code.  The solution for fixing the textarea issue that caused overflow was to add $t_out .= '</text'.'area>'.  I tried replacing </textarea> with that, but then my Submit and Reset buttons do not appear.  Everything after the $t_out .= '</text'.'area>' command cuts off the rest of the code wheile viewing the article.  Of course, the code editor is working fine now.  If I put it in the original way, the form will work, but I can't use the editor as it has overflow and the Save button no longer works when this happens.
