Need some scripting help

I need some help. I want to put together a page that people can apply for something, and have it sent to an e-mail can anyone help me?

5cakids do you think you could be a bit more specific, is this for a block a article, what do they have to apply for?

It would be for an article, I can get a block to point to the article but dont know how to make it go to an e-mail. It would be to apply for membership into a gaming clan
