Application mod

Hey i was looking around and i cant seem to find anything like this. I was wondering if some one can write this either as a mod to be installed or a scripted that can be entered into a block.

A lot of smf users from time to time need to make applications for clans/guilds or basic forms for others to fill. Usually me just make a forum post and ask people to fill it out as reply, but they always mess up the format. so i was wondering if some can write a mod/code for such a thing.

This is what i was thinking field wise:

• Out put: (location on the forum the result will be posted. If possible, dont know how it works with user groups, for it to be posted in a place where members usally cant post. Or if its not possible then option between a forum post or pm to admin defined user.     
• Output name: admins decide:
  Field                           rank(in order if multi fields are picked)
  User name <+>          <rank number>
  Admin defined name  <rank number>
  User define name       <tank number>
• Message icon (admin defines options)
  Admin defines icon <+>
  User defines icon    <+>
  User picks predefined icons by admin <+>     
• Question (admin defines)  <+add question, like a poll.> <+required (admin defines if required>   
• Answers (admin picks which):  <Text field> <Multi options, answer can be multi options. like a check box>   <Single option, like a male or female field>     

well thats my idea, any one care to take a shot at it ?
